| 標題 | Association between Coexisting Dementia and Mortality in Patients Receiving Oral Epidermal Growth Factor Receptor Tyrosine Kinase Inhibitors: A Nationwide Real-world Database Study |

| 概要 | This nationwide real-world database study investigated whether coexisting dementia is associated with mortality in patients with EGFR-mutated non-small cell lung cancer receiving first-line oral EGFR tyrosine kinase inhibitors (EGFR-TKIs). Using a Japanese administrative claims database, the authors identified 13,281 eligible patients treated between 2018 and 2024, of whom 282 (2.1%) had dementia. The primary outcome was 3-year all-cause mortality. Patients with dementia were older and showed poorer outcomes than those without dementia. The 3-year mortality rate was 41.9% in the dementia group versus 30.8% in the non-dementia group, and coexisting dementia was independently associated with higher mortality after adjustment (adjusted hazard ratio 1.35, 95% CI 1.06-1.73). In addition, medication adherence at 1 year was lower among patients with dementia. Among patients with dementia, preserved functional status was associated with lower mortality. These findings suggest that dementia is an important prognostic factor in patients receiving oral EGFR-TKIs and that maintaining functional status may help improve outcomes in this population. |
